Below are documents and information that are crucial to participation in the Give Kids A Smile! (GKAS) program.  Parents and school officials (nurses, teachers, principals, etc.) who will be sending or accompanying children to any of the participating locations in New Jersey are strongly encouraged to read the information in each of these documents.

Please note that every child must be accompanied by an adult that is either the child's parent, guardian or a school official.  Also, appointments for each child or groups of children must me made in advance directly with the participating location.
