State Board Notice on Incident Reports Nov 30, 2018 September 17, 2018 When dentists (and their office staff) complete an "Incident Report Form" about situations when a patient is removed to the hospital, the Board expects that the form will be typed and signed by the dentist. The current form is available here. As stated by the Executive Director of the State Board: "The document is a fill-able PDF, so it is able to have information entered into the various fields, saved, printed and signed. The form is required to be signed by the dentist who is reporting the incident. The Board also notes that the last field requests the disposition of the patient at the time the patient was discharged from the hospital - it is expected that the dentist attempt a follow up with their patient to provide this information to the Board."
